Please visit or call our main offices to know information on our enrollment process. Our office hours are from 8 AM - 4 PM, Monday through Friday. Please use that time to retrieve a packet from our offices. To begin the process we initially use the birthdate and home address to assess the perfect learning environment suited for the student. Here is a brief list of what is required to get your documents started. Click HERE for more information.
THE CHILD must be present (for all grades).
- Children entering Kindergarten must be 5 years old on or before September 1st, of the present school year.
- PROOF OF BIRTH (for children entering Kindergarten) one of the following:
- Original Recorded Birth Certificate (not Hospital Certificate)
- Baptismal Certificate
- Passport
- IMMUNIZATION RECORD (for all grades) indicating the following:
- DPT: 5 doses (Diphtheria, Tetanus, Pertussis-Whooping Cough) 4 doses OK if one was given on or after the child’s 4th birthday.
- POLIO: 4 doses. 3 doses OK if one was given on or after the child’s 4th birthday.
- HEPATITIS B: 3 doses for Kindergarten entrance entry.
- MMR (Measles, Mumps, and Rubella): 2 doses given on or after first birthday. Only 1 dose of mumps and rubella. Vaccines are required if given separately.
- VARICELLA: 1 dose or proof of documentation by medical provider that child had Varicella.
- HIB: 1 dose after 1st birthday for SRLDP entry.
- MANTOUX TB TEST: Required for children coming from a foreign country. Must indicate date given & dated results. If positive a chest x-ray with results must be provided.
- ORAL HEALTH ASSESMENT/WAIVER REQUEST FORM needs to be completed by dentist.
- TRANSFER if child is entering from another school.
- ADDRESS VERIFICATION (for all grades) rental agreement, current bank statement, current taxes, current utility bill (gas, water, electric) with name & address of parent/guardian. RENT & TELEPHONE RECEIPTS ARE NOT ACCEPTED.
- PARENT I.D. (photo and name).
